Insurance Information
We
have two areas of billing. The two entities are separate and distinct.
The first entity is our office... Associates in Dermatology &
Aesthetic Surgery (ADAS). This is where office
visits and office surgery is performed. The second
entity is our surgery center. Ponte
Vedra Surgery Center (PVSC). This is where we
perform more aggressive surgeries or certain
cosmetic procedures such as liposuction, laser
resurfacing or eyelid surgery.
It is
important to understand this because
you may receive two (2) statements if you are having
certain types of surgical procedures performed at
our "office & surgery center complex". In such
instances, the first statement would be
generated as an office billable service (physician
payment) and the second statement would be
generated as a surgery center service (facility fee
payment).

Ponte Vedra Surgery Center Our surgery center,
Ponte Vedra Surgery Center (PVSC) is
a separate and distinct facility. In terms of a surgery
center facility fee, A facility
fee may be charged to either you and/or your insurance carrier
if your case requires a complex repair, flap, or
skin graft to close the wound.
Please check with your insurance carrier or our
billing department for information on coverage,
deductibles and co-pay information. If your
insurance company is one that we do not participate
with, you will be responsible for the facility fee. For more

The above is a partial
listing meant to assist those seeking
care through our office. If your insurance
plan is not listed, please call the customer service number listed
on the back of your card to determine if we
are a network provider. The State of Florida does
have an open access policy for dermatology
care (5 visits within a twelve
(12) month period for managed care). Remember, it is your
responsibility to verify if a referral is
needed and to maintain a record of the
number of visits to our practice. Our office
